Asthma affects - порожает 5–8% of the population. It is characterized by recurrent episodes of dyspnoea - повторяющиеся эпизоды одышки, cough, and wheeze - хрипы caused by reversible airways obstruction - обратная обструкция дыхательных путей. Three factors contribute to airway narrowing - способствуют сужению дыхательных путей: bronchial muscle contraction - сокращение мышц бронхов, triggered by a variety of stimuli; mucosal swelling/inflammation - отек/воспаление слизистой, caused by mast cell - тучные клетки and basophil degranulation resulting in the release of inflammatory mediators - дегрануляция базофилов, приводящая к высвобожению медиаторов воспаления; and increased mucus production - увеличение продукции слизи. Assess oxygenation - оцените оксигенацию: oxygen saturation, arterial blood gases and blood pressure. Blood tests: FBC - full blood count - ОАК , urea - мочевина and electrolytes and creatinine, liver function test - функциональные пробы печени - ферменты печени, билирубин, ГГТ, щелочная фосфатаза, общий белок протромбиновое время, CRP - с-reactive protein - с-реактивный белок.
CXR - chest x-Ray - рентген грудной клетки: lobar or multilobar infiltrates, cavitation - каверна, or pleural effusion. Sputum for microscopy and culture. Urine - моча: check for Legionella/Pneumococcal urinary antigens. Atypical organism/viral serology (PCR sputum/BAL, complement fixation tests acutely - реакци связывания комплимента, paired serology - серологическое исследование парных сывороток). Pleural fluid may be aspirated for culture. Respiratory physicians may consider bronchoscopy and bronchoalveolar lavage if patient is immunocompromised or on ITU - intensive therapy unit - отделение интенсивной терапии. Открыть/Комментировать

Pneumonia

An acute lower respiratory tract infection associated with fever, symptoms and signs in the chest, and abnormalities on the chest x-ray.

Есть отличия в классификации.

Classification
Community-acquired pneumonia - внебольничная
Hospital-acquired pneumonia - госпитальная
Aspiration pneumonia - аспирационная
Immunocompromised patient pneumonia - пневмония у иммунокомпрометированных пациентов

Clinical features
Symptoms: Fever, rigors - озноб, malaise - недомогание, anorexia, dyspnoea, cough, purulent sputum - гнойная мокрота, haemoptysis, and pleuritic pain.
Signs: Pyrexia, cyanosis, confusion - спутанность сознания (can be the only sign in the elderly—may also be hypothermic)!, tachypnoea, tachycardia, hypotension, signs of consolidation (reduced expansion - уменьшенное расширение если дословано, но это отставание грудной клетки при дыхании, dull percussion - тпой перкуторный звук, increased tactile vocal fremitus - усиление голосового дрожания/vocal resonance - усиление бронхофонии, bronchial breathing), and a pleural rub - шум трения плевры.

Для озноба есть еще одно хорошее слово chills.

! - очень часто у пациентов старшего поколения, особенно коморбидных (а они всегда коморбидные:)) присутствуют только жалобы на усталость - fatigue и слабость faintness. При объективном осмотре часто можно найти симптомы и признаки указанные выше. Открыть/Комментировать

2022-05-03 10:00:21 The diagnosis of herpes zoster is typically clinical. Although herpes zoster is difficult to identify during the prodrome, the appearance of the typical exanthem aids in diagnosis. Testing is typically not needed, but it may be considered in patients with recurring lesions that are suspicious for herpes simplex, or in those with suspected zoster sine herpete, in which the virus causes pain without lesions. Testing may also be considered in atypical presentations, such as the widely disseminated lesions that may occur in immunocompromised patients. Testing is also helpful in differentiating herpes zoster from other vesicular dermatoses, such as contact dermatitis and dermatitis herpetiformis. Polymerase chain reaction testing of vesicle or other body fluids is preferred because of its high sensitivity and specificity.
